Copy of the invoice for the purchased additives. 4. Test report from an accredited laboratory for plastic objects as per the requirement of UAE Standard 5009/2009 including : (valid for 3 years) ? Abiotic Degradation Test reports ? residual material on the non-degradable fraction ? Ultimate Aerobic Biodegradability 5. Copy of Quality management system. According to ISO 9001. 6. Distributor ownership.( a template is attached, must be signed and stamped by the manufacturer) 7. Declaration of using a Plastic additive for each batch with the percentage of additives.(a template is attached) 8. Summary of your products.
